Insurance Information

As a courtesy extended to our patients, we work closely with you to maximize your insurance reimbursement for covered procedures. We will process both primary and secondary insurance claims. However, exact insurance payments are subject to carrier approval and limitations at the time the claim is processed, and the amount due our office is subject to change once the carrier has paid the claim.

To arrive at an “estimated” co-payment, we will contact your insurance carrier by telephone regarding plan deductibles, benefits, and any portion not covered by your insurance. Estimated co-payments are due at the time of treatment. If you prefer we obtain this information in the form of a written predetermination, please advise our office at the consultation. This will require four to six weeks for processing before treatment can proceed. Whether verbal or written EOB (Estimation of Benefits) are obtained, any balance remaining after insurance has processed is due upon receipt.

If you have any problems or questions, please contact our administrative staff at (850) 651-6882. We will make reasonable attempts to resolve insurance disputes on your behalf but, as health care providers, our relationship is with you and not your insurance company. Therefore, insurance payment disputes are ultimately the patient’s responsibility.

Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.
